Route: /dashboard/platform/intake. Needs canManageIntake.

How a form is issued

• requestAppointment creates an IntakeForm and returns a one-time access code.
• Staff can issue another with issueIntakeForm. The code is shown once. Only a hash is stored, and it expires after 7 days.
• The patient opens /intake#accessCode=…. The code is in the URL fragment, so it is not sent to the server in the page request.

Statuses

not_sent → sent → in_progress → submitted → needs_review or reviewed.

Review

Open a submitted form, read responses, then mark it reviewed or needs review with transitionIntakeReview(operationKey, intakeFormId, toStatus, expectedVersion, reviewNotes). Contact updates and consent submitted with the form are applied through submitIntakeForm.
